Discussion of patient concerns & reason for visit
Hands-on assessment, measurements & testing
Discussion of PT diagnosis & prognosis/goals & self-management education
Discussion of Findings & Goals (~10 min)
Session Scheduling, Payment Processing (~5 min)
Receipt of Treatment Plan (ASAP)
After your initial evaluation, you will receive an email summary of your PT diagnosis & prognosis, any key education points covered, and your comprehensive treatment plan:
Hyperlinked videos of rehab exercises
Weekly schedule (printout)
Key education & resources
Follow-Up PT Sessions
Individualized Treatment Session (~45 minutes) may include:
Self-management & other health education (e.g., at-home pain management strategies, holistic recovery tips, activity modification protocols)
Manual therapies (e.g., joint range of motion & mobilization, soft tissue work, neuromuscular re-education techniques)
Guided exercise & therapeutic activity
Clinical re-examination, when needed
Progress Discussion (~10 min)
Follow-Up Session Scheduling, Payment Processing (~5 min)
“Graduation Day”
Treatment/Plan Review & Receipt (~30 minutes)
Review and final adaptation of treatment plan — patient will receive copy of rehab routine & fitness recommendations, which they can use if flare-ups occur
Review of key self-management & other education & resources to maintain health status and results for the long-run!
Final Progress Discussion (~15 min)
Discussion of status, time to answer any final questions & address concerns
